What is Direct Primary Care(DPC)?
Direct Primary Care (DPC) is direct to consumer medical care, where patients pay a monthly membership fee to a medical practice for their primary care needs. There are no insurance bills, insurance co-pays, or deductibles. All routine and acute care visits are covered in the membership. The benefit to DPC is more personalized and accessible care, less out of pocket cost for you, and fewer constraints from commercial insurers.

​Quality healthcare is what drives us at Elevate Hydration. After over 6 years of experience as a RN in emergency care and med-surg I wanted to promote health and wellness on my own terms and my own time. So I chose to further my education as nurse and gained my Nurse Practitioner with a specialty in Family Care in October of 2023.
Preventative care is so important and what better way to do that than through vitamin therapy to heal the body and improve health. An intake form is needed prior to receiving any services to make sure you can receive our services. Please be on the look out for an email to the patient portal once you have booked your service.

What is needed prior to my appointment?
Do you offer gift cards or group package/parties for IVs or injections?
Yes we do!
We offer gift cards on our website.
We also can customize group packages at your location of choice. Please contact Elevate Hydration to schedule.
What are the payment options?
At Elevate Hydration we accept cash, debit card, and all major credit cards. We also offer Cherry as a payment option.
How old do you have to be to receive an IV service?
Under the age of 14 years old will require a consent form from their pediatrician prior to receiving any services. All children under the age of 18 years old will need a parents consent.
